BACKGROUND:Tacrolimus is a key drug in kidney transplantation (KT) with a narrow therapeutic index. The association between the tacrolimus metabolism rate and KT outcomes have not been investigated in large-scale multi-center studies.METHODS:The Korean Organ Transplantation Registry (KOTRY) datasets were used. A total of 3,456 KT recipients were analyzed. The tacrolimus metabolism rate was defined as blood trough concentration of tacrolimus (C 0 ) divided by the daily dose (D).RESULTS:The patients were grouped into fast, intermediate, or slow metabolizers by the C 0 /D measured 6 months after transplantation. The slow metabolism group was associated with a 2.7 ml/min/1.73 m 2 higher adjusted estimated glomerular filtration rate (eGFR) at 6 months (95% confidence interval (C.I.) 1.2-4.3, p=0.001), less acute rejection (AR) within 6 months (Odds ratio (OR) 0.744, 95% C.I. 0.585-0.947, p=0.016), and less interstitial fibrosis and tubular atrophy (OR 0.606, 95% C.I. 0.390-0.940, p=0.025). Fast tacrolimus metabolism affected the 6-month post-KT eGFR through mediation of AR (natural indirect effect (NIE) -0.434, 95% C.I. -0.856--0.012, p=0.044) and delayed graft function (DGF) (NIE -0.119, 95% C.I. -0.231--0.007, p=0.038).CONCLUSIONS:Slow tacrolimus metabolism was associated with better post-KT eGFR. Background/Aims:To investigate if BK virus (BKV)-specific T cell immunity measured by an interferon-γ enzyme-linked immunospot (ELISPOT) assay can predict the outcome of BK virus infection in kidney transplant recipients (KTRs).Methods:We included 68 KTRs with different viremia status (no viremia [n = 17], BK viremia [n = 27], and cleared viremia [n = 24]) and 44 healthy controls (HCs). The BK viremia group was divided into controller ( 3 months) according to sustained duration of BKV infection. We compared BKV-ELISPOT results against five BKV peptides (large tumor antigen [LT], St, VP1-3).Results:BKV-ELISPOT results were higher in three KTRs groups with different BKV infection status than the HCs group (p < 0.05). In KTR groups, they were higher in cleared viremia group than no viremia or BK viremia group. Within the BK viremia group, controller group had higher LT-ELISPOT results compared to noncontroller group (p = 0.032). Also, KTRs without BK virus-associated nephropathy (BKVN) had higher LT, St, VP1, and VP2-ELISPOT results than those with BKVN (p < 0.05).Conclusions:BKV-ELISPOT assay may be effective in predicting clinical outcomes of BKV infection in terms of clearance of BK virus and development of BKVN.

Li-rich Li_(1.2)Ni_(0.17)Co_(0.17)Mn_(0.5)O_2 cathode materials were synthesized by electrospinning technique with different polymers, and their structural, morphological, and electrochemical performances were investigated. It was found that the electrospinning process leads to the formation of a fiber and flower-like morphology, by using different polymers and heat treatment conditions. The nanostructured morphology provided these materials with high initial discharge capacity. The cycling stability was improved with agglomerated nano-particles, as compared with porous materials.

In present study, fluorine substituted Li_(1.23)Ni_(0.13)Co_(0.14)Mn_(0.56)O_(2-z)F_z (z=0-0.20) are synthesized by simple and low-temperature co-precipitation process and observed enhancement in their electrochemical properties are investigated as function of fluorine substitutions. ~(19)F MAS NMR analyses revealed the presence of fluorine as metal fluoride on the surface, and as oxy-fluoride in bulk Li_(1.23)Ni_(0.13)Co_(0.14)Mn_(0.56)O_(2-z)F_z. Li_(1.23)Ni_(0.13)Co_(0.14)Mn_(0.56)O_(2-z)F_z (z = 0.15) showed improved cycling performance and reduced cell impedance as a result of the fluorine doping. In addition, fluorine substitution controlled the amount of stabilizing transition metals in the lithium layers of spinel-like regions, which in turn resulted in countering the capacity loss and voltage decay compared to Li_(1.23)Ni_(0.13)Co_(0.14)Mn_(0.56)O_2.

Background: A decrease in the number of tissue eosinophils is known to reflect the malignancy potential of neoplastic lesions and even prognosis. Increased levels of the chemokines CCL11 and CCL24 in serum and tissue are also known to have diagnostic value as serum tumor markers or prognostic factors. The aim of this study was to evaluate the correlation between the degree of tissue eosinophilia and the expression of these chemokines in the glandular and stromal cells of colorectal neoplastic lesions ranging from benign to malignant tumors. Methods: We counted the number of infiltrating eosinophils in neoplastic lesion tissue and we evaluated the expression of CCL11 and CCL24 in glandular cells and stromal cells by immunohistochemical staining. Results: The results showed that the number of eosinophils decreased significantly and the expression of CCL11 and CCL24 in glandular cells decreased with tumor progression, whereas the stromal expression of CCL11 and CCL24 appeared to increase. Conclusions: The discrepancy in CCL11 and CCL24 expression between glandular cells and stromal cells might shed light on how colorectal cancer evades the immune system, which would enable further development of immunotherapies that target these chemokines. Further research on eosinophil biology and the expression pattern of chemokines in tumor cells is needed.

The morphological changes occurred with temperature increase from 500 °C to 700 °C. Scanning electron microscopy showed that flowcr-likc Li_(1.2)Ni_(0.17)Co_(0.17)Mn_(0.5)O_2 after heat treatment at 600 °C was composed of nanoplates with an open porous structure. Flower-like electrode with well-organized porous structure can be attributed to the favorable shape to facilitate the diffusion of lithium ions. Flower-like Li_(1.2)Ni_(0.17)Co_(0.17)Mn_(0.5)O_2 microstructures showed a high discharge capacity of 235 mA h g~(-1) during the first cycle compared to other electrode materials.

Spontaneous renal artery dissection is a rare disease and an uncommon cause of renal infarction. The patient was a man who presented to the emergency room with sudden-onset right flank pain. Computed tomography revealed right renal infarction; thus, anticoagulation was initiated. Renal angiography revealed luminal narrowing of the segmental artery to the superior pole of right kidney without a dissection flap or false lumen. We stopped anticoagulation due to a lack of evidence of thrombi or luminal narrowing of the dissected vessels. When patients present with acute flank pain, it is important to suspect renal infarction and to perform a correct diagnostic workup, even when the patient shows normal urinalysis results and a normal LDH value.

Portal vein thrombosis is an uncommon but an important cause of portal hypertension. The most common etiological factors of portal vein thrombosis are liver cirrhosis and malignancy. Albeit rare, portal vein thrombosis can also occur in the presence of local infection and inflammation such as pancreatitis or cholecystitis. A 52-year-old male was admitted because of general weakness and poor oral intake. He had an operation for colon cancer 18 months ago. However, colonic stent had to be inserted afterwards because stricture developed at anastomosis site. Computed tomography taken at admission revealed portal vein thrombosis and inflammation at colonic stent insertion site. Blood culture was positive for Escherichia coli . After antibiotic therapy, portal vein thrombosis resolved. Herein, we report a case of portal vein thrombosis with sepsis caused by inflammation at colonic stent insertion site which was successfully treated with antibiotics.

The Li_(1.2)Ni_(0.17)Co_(0.17)Mn_(0.5)O2nanofibers were synthesized by a simple electrospinning process. Scanning electron microscopy (SEM) and transmission electron microscopy (TEM) showed that electrospun nanofibers with small particle size of 10—30 nm were formed. It was found that the electrospinning process leads to the formation of an effective conducting nanofiber, which provides improved intercalation kinetics. The eletrospun Li_(1.2)Ni_(0.17)Co_(0.17)Mn_(0.5)O2 nanofibers showed a high discharge capacity of 256 mA h g~l during the first cycle. In particular, the electrospun Li_(1.2)Ni_(0.17)Co_(0.17)Mn_(0.5)O2 nanofiber sample exhibited excellent rate capability when compared to the co-precipitated Li_(1.2)Ni_(0.17)Co_(0.17)Mn_(0.5)O2 particle sample.

Nitrobenzene is a poisonous agent, not commonly encountered in clinical practice, which belongs to the aniline dyes. Ingestion of nitrobenzene may cause methemoglobinemia, a condition in which the iron in hemoglobin is oxidized from the ferrous state to the ferric state, resulting in the inability to transport oxygen. A 41-year-old man presented with the clinical features of methemoglobinemia after drinking nitrobenzene. The patient was treated conservatively with intravenous methylene blue. We report a case of acute methemoglobinemia due to ingestion of nitrobenzene.
